~Apply to as many programs as possible. Apply broadly all across the USA.
~I believe you do need strong grades, USMLE scores, and strong dermatologist LOR's to match. AOA, Research, Publications, and Graduating from a TOP 20 Med School tremendously improves your application. However, I'm proof that you do NOT have to be AOA or come from a Top 20 Medical School to match in the ultra-competitive field of Dermatology =)
~Ask dermatologists if they have any interesting case reports or review papers that you can help them write up. This is a good way to work with and get to know a dermatologist, show them you're a hard worker, and hopefully get a strong LOR and publication out of it. But start early if you want to add a publication to your CV. All 2 of my Derm publications took over 1 year to get accepted for publication (excruciatingly painful waiting game)! ~GOOD LUCK!
